Care Coordination for Deaf and Hard of Hearing Adults and Youth
WHOLE ME, Inc.’s Care Coordination Program is designed to identify the unique needs of Deaf and hard-of-hearing individuals and create a culturally appropriate comprehensive plan of care that includes healthcare, school, and community. WHOLE ME will partner with health and service providers in order to achieve optimal health, wellness and life goals. Our program provides person-centered, holistic coordination of services that ensure high-quality care that is integrated and supports the needs of Deaf and hard-of-hearing individuals.
To help meet the needs of Deaf and hard-of-hearing individuals, your coordinator will assist you in the following ways:

Comprehensive Care Management

Help you create your Plan of Care and help you get started

Help you update your Care Plan

Review your current service needs (Your providers and support services, your goals, and your engagement activities)
Care Coordination and Health Promotion

Provide you with education and support for health and wellness

Coordinate and arrange for additional needed services and ensure you have what you need to follow your Care Plan
Comprehensive Transitional Planning

Transition planning includes getting help when you discharge from a hospital or other institution such as a residential/rehabilitation settings

Assist you and your family prepare for life changes such as changes in needed services fit to your needs
Individual and Family/Advocate Support

Educate about support and self-help resources to help increase knowledge, engagement, and independence

To assist you how to support your ability to take care of yourself or those in your care

Share additional information about educational resources and support services
Referral to Community and Social Supports

Your coordinator will explain how they will assist in managing referrals, access, engagement, follow-up and coordination

Advise you, your family, and your support of available resources

WHOLE ME does not authorize services. You will work in partnership with your care coordinator to identify support and services based on your needs. Your coordinator will be your primary contact to help coordinate these services. You should have fewer gaps in service and better coordination between all of your providers.

Appointments:
​
Appointments accepted Monday, Wednesday, and Thursday 10am-5pm, including outside of normal work hours if needed. This can be in-person or on virtual platforms. Walk-Ins accepted: Tuesday and Friday from 11am-2pm for immediate services.
